Position: Quality Assurance Specialist (Home Health)

Location: Burbank, California

Responsibilities:

• Conduct comprehensive quality assurance reviews of all aspects of our home health services, including patient care plans, documentation, and adherence to regulatory standards.
• Develop and implement quality assurance protocols and processes to ensure compliance with industry regulations and best practices.
• Collaborate with interdisciplinary teams to identify areas for improvement and implement corrective actions to enhance the quality and efficiency of our services.
• Perform regular audits and inspections to assess the effectiveness of our quality assurance initiatives and identify opportunities for optimization.
• Provide training and guidance to staff members on quality assurance policies, procedures, and documentation requirements.
• Investigate and address any quality-related concerns or incidents in a timely and thorough manner, implementing preventive measures to mitigate future risks.
• Responsible for the review of all documentation submitted to the mental health client record, including (but not limited to), intake documents, assessments, Client Treatment Plans (CTPs) and Problem Lists, progress notes, change of diagnoses, outcome measures, medication support/psychiatric documents, and correspondence.
• Conduct reconciliation, document-specific reviews, and full chart reviews, and provide feedback on needed corrections, trends, areas of needed training, and missing information to Director of Quality Assurance.
• Responsible for maintaining any relevant licensure/registration, education requirements for CEUs, and current knowledge of client services, documentation requirements, best practices, coordination of care, risk assessment and planning, and intervention strategies via continued education, workshops, seminars and/or specialized trainings to remain current and proficient in factors/interventions relevant to the client and family population.

Qualifications:

• Bachelor's degree in nursing, healthcare administration, or a related field (preferred).
• Minimum of 2 years of experience in quality assurance or a similar role within the healthcare industry, with specific experience in home health care preferred.
• Strong understanding of regulatory requirements and accreditation standards relevant to home health care services (e.g., Medicare, Medicaid, Joint Commission).
• Excellent analytical skills with the ability to identify trends, patterns, and areas for improvement.
• Exceptional communication and interpersonal skills, with the ability to effectively collaborate with multidisciplinary teams and communicate complex information clearly and concisely.
• Detail-oriented mindset with a commitment to accuracy and thoroughness in all aspects of work.
• Proficiency in using electronic health record (EHR) systems and other relevant software applications.
